This form is designed to help us evaluate whether your child is a suitable candidate for our social skills groups. We aim to ensure that each participant will benefit from and contribute to the group dynamic. Please make sure all information is accurately provided.
Please note that completing this form does not guarantee that your child will be a candidate for the open house or an invitation to participate in our social skills groups. Potential candidates will be notified with more information by email or a phone call from one of our staff.
